Product notifications - Registration

Companies must send to CIAV the following information  related to EACH PRODUCT:

• BRAND NAME AT THE PORTUGUESE MARKET

• INTENDED USE

• 100% CHEMICAL COMPOSITION

  • Product Information Sheet

• PACKAGE CAPACITY

• COPY OF THE LABEL

Its also necessary to send a LIST with the name of all products to register and the IDENTIFICATION and CONTACTS OF THE DISTRIBUTOR in Portugal.

All documents should be sent by e-mail to ciav.tox@inem.pt .

CIAV will send back by e-mail the respective “Registration Certificate”.

PRODUCT SHEET - INSTRUCTIONS

1. Brand name at the Portuguese market

2. Product description

• Intended use (Ex: Manual detergent; Diswashing detergent …)

3. Package

• Type – Bottle; spray; box …

4. Information about the distributor

5. Market issue

6. Product reference

• Usually a number or a letter-numerical reference which facilitate the product identification.

7. Chemical composition

• 100% chemical composition, with all the ingredients and each percentage in the formula. If the number of ingredients exceed the lines available please use a second sheet, with the reference to the first one.

8. Information about the manufacter

NOTE–   Please send a list with the name of the products to register and a copy of the label at the Portuguese market.
